Hospitals outline progress since IOM report on patient safety

 Hospitals have made great strides over the past 10 years in patient safety, transparency and the way they work, AHA President and CEO Rich Umbdenstock said during a panel discussion today. The Health Research and Educational Trust event marked the 10th anniversary of the 1999 Institute of Medicine report "To Err is Human." Noting that "there is always more that we can do," Umbdenstock cited national programs, collaborations among clinical experts and internal initiatives as ways hospitals continue to reduce errors and improve quality and patient safety.
